We print our 1095C forms on an 8 x 11 front and back. Our developer modified the form to be two pages. Front includes the 1095C instructions on 2/3 of the page and an address label that appears in the lower 1/3 so when folded, the address appears through the clear window on the envelope. On the back top half is the first page of 1095C with Parts I, II, and III, then the bottom half is the continuation of Part III if they have additional dependents. This will save on printing and mailing costs. Below is an example.
